As part of our Employee Growth Plan, the following self-evaluation form has been developed to help prepare you for your individual one-on-one review meetings with our management team. We are very excited about your evaluation which will help us recognize your achievements, strengths and contributions to the company as well as help identify areas where improvement is needed by employee and management. Please ensure that you take the time to review each question and answer to the best of your ability. 1) ATTITUDE - Employee displays a positive attitude with employer and co-workers and is always willing to take on new challenges and learn more regardless of difficult situations. Employee is able to work with project related issues and is able to meet difficult project demands with a positive attitude and resolve problems as they arise. Employee has an overall positive attitude and demonstrates leadership and the ability to be a role model (i.e., courteous, travel expenses, time sheet integrity, hours worked etc.)
