As part of our Employee Growth Plan, the following self-evaluation form has been
developed
to help prepare you for your individual one-on-one review meetings with our management team. We are very excited about your evaluation which will help us recognize your achievements, strengths and contributions to the company as well as help identify areas where improvement is needed by employee and management.
